I had a transvaginal ultrasound a week and a half ago, at which time my doctor found five 'small' cysts on my right ovary and two 'very large' ones on my left ovary. My doctors think they're blood-filled endometriomas (chocolate cysts). (I had the US for infertility investigation.)
Can endometriomas form at any stage (1 through 4) of endometriosis? I've read on a couple sites that fertility can be a problem at any stage, but is most commonly an issue in stages 3 and 4. But what about the chocolate cysts?
I'm really curious about what stage I am, and obviously I want it to be as low as possible, but I can't wait for three days until I see the surgeon for my preop appointment (having a lap in 2 and a half weeks and they may have to remove my left ovary)!
For those of you who have had endometriomas: What stage are you? I know that the only way I can definitely know what stage I am is through surgery, but I'm just curious about what my likelihood of having a mild case of endo is.

Technically, I believe any endometriotic implant is an endometrioma...doesn't have to be on an ovary, although when endometriomas occur on the ovaries, they are more commonly referred to as "chocolate cysts." I had endometriomas on my intestines, and my intestines aren't my ovaries. I know this isn't the answer you were looking for--but no one else seems to be saying anything.
I think the chocolate cysts happen in late stage endo, but I'm not sure. They are filled with old blood, and I am not sure, but I think I read somewhere that they are hormonally inactive??? Again, NOT sure. I know that sometimes endometriosis gets on the ovaries and it's that red color and splotchy, and sometimes it gets on there and leads to the chocolate cysts. That's about all I know. Sorry.

Thank you. Yeah, I was specifically talking about chocolate cysts. I'm obviously hoping to have a mild case of endo (because of fertility), but it's not looking good.

I have endometriomas (chocolate cysts) and I am on stage IV. I believe you can have chocolate cysts at any stage but probably they are more abundant at higher stages (3-4)

Apple -
Do you notice a lot of pain that you think is associated directly with the cysts? I have cramping all over my abdomen (from above where my ribs end to my hips), but I particularly have stabbing pains on my left side (front and back). My left side apparently has large cysts.

I believe that some of the pain that I have is related with my cysts, since when I have the pain I kind of feel them. I have stabbing pain on both side during my period. Outside my period I also get pain on my right side. Front and back, similar to what you are describing. I also get some pain right below my diaphragm ( I do have endo implants on this area as well).

In addition to my side and back pain (and pelvic area pain), I'm also having bad pains right below my lowest ribs in the middle. It hurts especially bad whenever I eat or drink anything, so much so that I've started skipping meals even though I'm really hungry.

A while ago I had the same, some days was impossible for me to eat or drink. I had terrible cramps on that area as well. Lately I am better regarding these particular symptoms, this is After I changed my diet and could be that acupuncture/Chinese herbs have helped as well.

Apple - Did you find anything that alleviated the pain? It's really difficult to eat anything, and I'm feeling sicker and sicker. I feel nauseaous all the time (I know I'm not pregnant or anything though ) My surgery to remove the cysts isn't for another two weeks though. I did find that a heating pack helps the pain a little bit, but it still doesn't make it so that I can eat or drink. I also read yesterday that pain with eating can be caused by endo adhesions on the small intestines.

Apple - sorry, when I just replied a minute ago I hadn't seen that part about the herbs and accupuncture. Maybe I'll ask my mother-in-law about accupuncture, since she goes to an accupuncturist frequently. What kind of diet changes are helpful for the pain?
